Publication of Names of Apparent Owners of Abandoned Property
Madison County

I. Mandatory Requirements
The Illinois Uniform Disposition of Unclaimed Property Act requires the Office of the
Illinois State Treasurer to notify Illinois residents who appear to be owners of Abandoned
Property of that fact. To this end, the Treasurer is requesting bids from Vendors which
publish an English Language newspaper of general circulation in Madison County,
Illinois, to publish the names and addresses of persons appearing to be owners of
Abandoned Property with last known addresses in Madison County in Illinois.
The approximate number of such names and addresses is in a range of 2,900 to 3,050.
The information will be forwarded to the successful Vendor along with a layout for the
front panel which may include text, photographs, graphics and inquiry form.
Names are to appear in bold type.
Vendor shall be given data supplied by the Treasurer, which must appear along with the
names and addresses of the apparent owners of Abandoned Property in the Sunday,
February 13, 2005 edition of Vendor’s newspaper.
Vendor shall provide the Treasurer with two (2) tear sheets reflecting the name of
Vendor’s newspaper, date of publication and the “NOTICE OF NAME OF PERSONS
APPEARING TO BE OWNERS OF ABANDONED PROPERTY” and return it to the
Treasurer along with any material provided to Vendor relative to the listing of such
names of apparent owners of Abandoned Property.
The Response to the Request for Proposals must also contain the following information:
A fully executed Financial Interest and Potential Conflict of Interest Disclosure Form
(Attached);
A fully executed Contract and Procurement Related Disclosure Form (Attached); and
A fully executed State Certification Form (Attached).
