The collection of this information stems from the Commission's authority under Section 13-407 of the Illinois Public Utilities Act. The Worksheet
entitled "Confidentiality" provides detailed information as to the treatment to be accorded the data that carriers file.
Do not insert or delete columns or rows, move cells, or edit text or
cells outside the cells provided for data entries. Files that cannot
be opened in Excel 2003 or whose structures have been altered
will be returned for refiling. If you cannot adhere to these
standards, please call ICC staff for assistance.

All incumbent local exchange carriers ("ILECs") and local exchange carriers certificated within the state of Illinois must complete the
Cover Page, the Company Identification Information, and Charts 1A, 1B, 1C, 1D, 1E and 2. ILECs must complete Charts 3 and 4.
Carriers that are certificated to provide local exchange service within the state of Illinois and broadband service providers within the
state of Illiniois must also provide, per Chart 5, a copy of the Form 477 that includes information for Illinois as of December 31, 2006
that is to be filed with the FCC on or before March 1, 2007. 911 Database providers are required to complete Chart 6. Carriers that
are certified to provide service, but that do not currrently offer either local exchange service or facilities based broadband service
within the state of Illinois may simply respond with a letter or e-mail to that effect. ILECs and their affiliates should submit separate
forms for incumbent and non-incumbent operations.
Reporting carriers must submit data for December 31, 2006 on or before March 1, 2007.
