Enclosed you will find the Illinois Risk Watch application packet. It contains:
  Risk Watch Informational Brochure
  Community Application
  Memorandum of Understanding (This is the most important item in the
packet because it lists the tasks that your community must agree to if
chosen as a champion site.)

Please carefully review with everyone the items your coalition will have to agree to if
chosen as a champion site.

The Illinois Risk Watch
Management Team will review each of the applications and
notify you of our decision. If your community is not selected this time, we ask that
you fill out another application for the next school year.
Again, we thank you for your interest in becoming a Risk Watch community. Should
you have any questions or need more pamphlets, please feel free to contact any one of
the state team members. Anyone of us will be willing to visit your community and
discuss the Risk Watch project with your team members.
